ECDS long format

This notebook provides the methodology for creating long format ECDS diagnosis, invesitgation and treatment tables. The long format dataframes which are produced provide the SNOMED-CT codes, qualifiers and descriptions (for the diagnosis table), and the SNOMED-CT codes descriptions and dates (for the investigation and treatment tables).
